2. Experience and Training.
Kathy has practiced as a classical homeopath in MN since 1999. She graduated from Northwestern Academy of Homeopathy, a three year program in Minneapolis, MN in June, 2000, and from the Luminos Homeopathic Master Clinician Course in Minneapolis, Minnesota in 2003.

She has completed training in Cranial sacral Therapy I and II, Pediatric Cranial Sacral Therapy, Somato-Emotional Release I and II, Advanced Cranial Sacral Therapy, Heart Centered Therapy I and II and Equine Cranial Sacral Therapy I with Upledger Institute, Inc. She has incorporated Cranial Sacral Therapy into her practice since June, 2003.

She has received her Masters in Spiritual Peacemaking and is an Ordained Minister from the Seminary of Spiritual Peacemaking in Talent, Oregon in 2004. She has accomplished Reiki Master Teacher certification in Usui/Japanese/Tibetan Reiki System; trained by Dr. Philip Vande-Reit, PHD,MPA after 2 intensive training seminars on June 24, 2001 and July 27, 2003. Reiki has been incorporated into her practice in July, 2001. Kathy has taken Shamanic Training courses with Bill Brunton of the Foundation for Shamanic Studies in Minneapolis, MN from 2004-2007.

Prior experience includes 30 years in the health care field. Prior training includes Bachelor of Science Degree, Specialty in Health Arts in 1985 from College of St. Francis, Joliet, Illinois; Associate of Applied Science Degree, Specialty in Nursing in 1978 from St. Mary’s Junior College, Minneapolis, MN. She is currently licensed as a Registered Nurse in both MN and AZ.

 

3. Arizona has passed a new law (Senate Bill 1236; ARS: 32-2911*) see attached link or (www.azhomeopathbd.az.gov/statutes.htm). This law provides an exemption to the practice of classical homeopathy for classical homeopathic practitioners in the State of Arizona if they meet the following criteria:

·         Provide Treatment of the "Spiritual" Vital Force

·         Use Classical Hahnemannian Principles in Treatment

·         Use Diluted Homeopathic Medicines (potency 6X or greater) Prepared Using HPUS Guidelines

4. Fees. My fee schedule is as follows:

  • Initial  Vital Force Consultation (1 ½-2 hours) $185 (includes remedy)
  • Follow up Consultation (30 minutes) $45 (does not include remedy)
  • Telephone follow up (30 minutes in place of office visit) $45 (does not include remedy)
  • Telephone consult ( 5 to 15 minutes) $15 (does not include remedy)
  • Remedy Charges: C potencies Start at $10 for lower potencies, ask practitioner for prices as may vary with cost from manufacturer (does not include shipping.)
  • LM potencies Start at $15 (does not include shipping)
  • Reiki treatments (45minutes-1 ½ hours) $65
  • Cranial Sacral Therapy, Shamanic Healing, Heart Centered Therapy ( 45 minutes- 1 ½ hours) $75

Fees are payable at the time of service. If you are unable to pay the full fee at the time of service, a payment plan can be arranged. This plan must be agreed to in writing prior to the provision of services. In order to receive services, you must be current with your payment plan arrangement. I do not handle insurance claims; however, a receipt will be provided to you should you wish to file a claim with your provider. I do not accept Medicare, Medical Assistance or General Assistance Medical Care.

5. Change in service or charges. You have the right to reasonable notice of changes in services or charges, and I will provide prior notice of any changes.

6. Brief summary of my Theoretical Approach. Homeopathy is a systematic form of therapy that stimulates the body’s innate ability to heal. Symptoms guide the homeopath to the needed remedy based on the principle of “like cures like”, meaning that, if a substance can cause symptoms in a healthy person, then it will stimulate self-healing of similar symptoms in a sick person. Homeopathic remedies are primarily made from plant, animal and mineral sources. Classical homeopaths use only one remedy at a time in the smallest amount needed to stimulate the body’s healing mechanism. The goal is to produce gentle, effective healing on all levels: mental, emotional, energetic and physical.

Reiki is a hands on healing technique that makes use of transmitted healing energy of a high frequency. This energy is Universal Life Force Energy or spiritually guided life force energy. The Reiki energy flows through the practitioner and out through the practitioners hands and into the client. No matter where the hands are placed, the Reiki energy will seek out and reach the area of the body where the energy is most needed. The Reiki treatment provides specific energy to the source of the problem, or unblocks the vital force in a specific organ.

Cranial Sacral Therapy is gentle, hands on therapy used to evaluate and monitor the rhythm of the cranial sacral system to detect potential imbalances and restrictions. The therapist uses delicate manual techniques to relieve undue pressure and release the problem areas of the brain and spinal cord, resulting in a central nervous system free of restrictions. It is also used as a preventative measure for its ability to bolster disease resistance and is effective for wide range of problems associated with pain and dysfunction.

7. Assessment and Recommendations. You have the right to complete and current information concerning my assessment and recommended service, including the expected duration of the services to be provided. If you have any questions, please ask.

8. Courteous Service. You may expect courteous treatment and to be free from verbal, physical or sexual abuse by the practitioner.

9. Confidentiality. Your records and transactions with this office are confidential. This information will not be released unless you authorize release in writing, or unless release is required by law.

10. Records. You are allowed access to records and written information from records in accordance with section 144.335 of Minnesota Statutes. See attachment “Access to Health Records Practices and Rights” notice from MN Dept. of Health.

11. Other Community Services. Other similar services may be available in the community.  You may ask your practitioner and she will provide this information to the best of her knowledge.

12. Selecting and Changing Practitioners. You have the right to choose freely among available practitioners and to change practitioners after services have begun, within the limits of health insurance, medical assistance or other health programs. If these services are covered by your health insurance, medical assistance plan or other health program, you should direct all questions about coverage to your insurance provider.

13. Coordinated transfer. If you change practitioners, you have the right to our assistance in coordinating this transfer to another practitioner.

14. Right to Refuse Service. You are free to refuse services or treatment, unless otherwise provided by law.

15. No Retaliation. You may assert your rights described in this Client Bill of Rights at any time without retaliation.
